This position will provide counseling and case management services with the purpose of increasing treatment adherence outcomes to HIV-infected patients in the Infectious Disease Clinic. The Adhe! rence Counselor will work with an interdisciplinary team comprised of primary social workers, medical providers, a psychiatrist, mental health counselors, nurses, a Pharm-D, clerical staff and volunteers. Work as a key team member within the CRCS Prevention for Positives Program, providing compassionate counseling services and coordinating other services needed for HIV/AIDS positive individuals. ESSENTIAL FUNCTIONS: â?¢Elicit referrals from doctors, physician assistants, nursing staff, patients, social workers, hospital staff, etc. â?¢Collaborate with the interdisciplinary team to elicit referrals and develop treatment plans with the patient. â?¢Provide patients with education and counseling related to HIV and other sexually transmitted infections; transmission risks; HIV/AIDS literacy; sexual health literacy; and risk-reduction practices. â?¢Monitor patientsâ?? health status, compliance with medical appointments, and adherence to medication regimens, and monitor t! reatment recommendations from the medical provider on an ongoi! ng basis.. â?¢Provide referral and linkages to ancillary services such as pharmacy, mental health, and substance abuse services. â?¢Assess patientâ??s psychosocial needs on an ongoing basis. â?¢Provide crisis intervention (or appropriate referrals) as needed. â?¢Document pertinent face-to-face client and client-related contacts. â?¢Complete quarterly reports with caseload outcomes based on a culmination of client-level data. â?¢Participate in regularly scheduled staff meetings. â?¢Participate in mandatory annual organizational trainings, trainings related to HIV/ AIDS, and other trainings targeted at the populations being served in the ID Clinic. â?¢Be responsible for contractual obligations re quarterly reporting and data entry into CAREWARE data base.
Minimum Qualifications
â?¢Excellent interpersonal skills. â?¢Excellent oral and written communication skills. â?¢Knowledge of HIV/AIDS and HIV/AIDS care network. â?¢Knowledge and sk! ill to provide counseling and education for HIV/AIDS infected individuals and their identified families. â?¢Ability to work independently under minimum supervision. â?¢Strong attention to detail. â?¢Experience with data collection/entry and familiar with Quality Improvement Program. â?¢Ability to work and communicate effectively with individuals from culturally and socioeconomically diverse backgrounds. â?¢Must have the ability to meet specific deadlines and coordinate multiple tasks. â?¢Must have knowledge of existing local social services systems (i.e. food banks, Departments of Social Services, substance abuse treatment resources, etc.) and assist patients as needed with referrals to those resources. â?¢Graduation from an accredited college or university with a Masterâ??s degree in Social Work, Human Service or related field or an equivalent combination of college and related experience. â?¢Experience in social work/case management. â?¢Working knowledl! ge of HIV infected populations preferred.
