INTENSIVE IN-HOME COUNSELOR
Dominion Day Services seeks a Mental Health In-Home Case Manager/Child Counselor to improve the quality of life for clients with severe behavioral problems that are at risk of being removed from their homes. As an In-Home Case Manager, you will work within patients homes to assist their family with defining the goals for their child and strive to provide the least restrictive environment to foster personal growth. This ! position will require you to develop relationships with family! members, become invested in their childs future and become an advocate for their client. You will work with the childs community service providers, teachers, principals, lawyers, probation officers and psychiatrists. The In-Home Case Manager should be self-motivated, creative, able to think on their feet and to easily adapt to any situation.
You will work with children in their homes to help them develop social and family relationships, construct an individualized behavioral modification plan for them that will improve familial relationships and stabilize their home environment, and offer individual counseling to the child and their family, provide family therapy and perform case management services. You will also focus on the following:
Providing crisis intervention services 24 hours a day, 7 days a week
Tracking all individualized service plans and paperwork
Managing paperwork and meeting deadlines
Attending staff meetings and communicating with super! visors
Discussing updates, progress and concerns with supervisors on a weekly basis
Coordinating with community service providers
SCHOOL BASED LEAD COUNSELOR:
- Provide for the coordination of admission procedures for each client.
- Provide for the coordination of transportation for each client.
- Implement individualized behavior modification program.
- Attend all staff meetings and trainings.
- Develop and implement all therapeutic group activities.
- Adhere to all policies and procedures and complete other tasks deemed necessary for the successful operation of the program.
- Complete all required documentation which includes, but is not limited to, the tracking of all individualized behavior programs and related paperwork i.e. (point totals, outcome tracks, discharge planning.

To be considered for these positions, you must be a Qualified Mental Health Professional and have passion for helping people. The exceptional candidate will also have:
Bachelors degree with a concentration in psychology, social work, human services, special education or counseling; Masters degree preferred but not required
In-Home Counselor: Minimum 1 year experience working with children with SED diagnosis and providing therapeutic and case management services to children who are at risk for out-of-home placement. School Based Counselor: one year of clinical experience working in day treatment , similar programs for serious emotionally disturbed children, or relevant clinical experience working with children or adolescents in the mental health field.
