The duties and responsibilities of this position are complex due to the mission of the Hampton Veterans Affairs Medical Center. The Medical Records T! echnician (Fee Coder) is responsible for performing a quality review of patient care documents and assigning codes specific for the type of care provided. The basic functions of this position includes, but are not limited to:
Assign codes to documented patient care encounters (inpatient and outpatient) covering the full range of health care services provided by the VAMC.
Applies advanced knowledge of medical terminology, anatomy & physiology, disease processes, treatment modalities, diagnostic tests, medications, procedures as well as the principles and practices of health services and the organizational structure to ensure proper code selection.
Selects and assigns codes from the current version of several coding systems to include International Classification of Diseases-Clinical Modification (ICD-9-CM and ICD-10-CM), Diagnostic and Statistical Manual of Mental Disorders (DSM), Current Procedural Terminology (CPT) and HCPCS.
Adheres to accepted coding! practices, guidelines and conventions when choosing the most ! appropriate diagnosis, operation, procedure, ancillary, or Evaluation and Management code to ensure ethical, accurate, and complete coding. Also applies codes based on specific guidelines to diagnoses, procedures, and other criteria (both inpatient and outpatient settings) used to classify patients under the Veterans Equitable Resource Allocation (VERA) program.
Performs a comprehensive review of the patient record to abstract medical, surgical, ancillary, demographic, social, and administrative data to ensure complete data capture.
Assists facility staff with documentation requirements to completely and accurately reflect the patient care provided; provide technical support in the areas of regulations and policy, coding requirements, resident supervision, reimbursement, workload, accepted nomenclature, and proper sequencing.
Directly consults with the professional staff for clarification of conflicting or ambiguous clinical data.
Utilizes the! facility computer system and software applications to correctly code, abstract, record, and transmit data to the national VA database in Austin.
Orients and instructs new personnel and/or students from affiliated health information or medical record technology programs, at the direction of the supervisor, on unit operations, coding, abstracting, and use of an electronic medical record.
Performs other directly related duties as assigned.

Five Medical Record Technician, GS-0675-8 positions are currently available in the Health Administration Sciences Service, Hampton VA Medical Center. The normal tour of duty is Monday through Friday, 7:30 a.m. to 5:00 p.m. There may be some instances where the incumbent would be required to work after 5:00 p.m., or have the flexibility to work outside the normal tour of duty which may include weekends and holidays. You must be a U.S. citizen to qualify for this position. Hampton VA employees will receive first consideration.
Medical record technicians (MRT) organize, process, and maintain medical records and health information. They assemble, review, analyze, code, abstract, maintain, compile, and extract medical record and healt! h data. MRTs check medical records for completeness, accuracy, and compliance with requirements of regulatory and accrediting agencies. Some MRTs maintain clinical registries and work to meet the standards of regulatory and accrediting agencies. Typically MRTs work under the guidance of the Medical Record Administrator. In the health care community MRTs are known by a number of titles depending on job function. Terms may include health information technician, coder, tumor registrar, or registry technician.
NOTE : The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the step at which the candidate will be placed in that specific grade. The salary is based on the grade and step approved for the selected candidate.

a. Citizenship. Citizen of the United States . (N! on-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g, this part.
b. Experience
(1) Experience. Two years of experience that demonstrates the applicantÂ's ability to perform the work, or provides familiarity with the work. Six months of the required 2 years of experience must have provided the knowledge, skills, abilities, and other characteristics (KSAs) needed to perform MRT work.
GRADE DETERMINATIONS GS-08:
In addition to meeting the basic requirements applicants must have a minimum of one year (52 weeks) of experience equivalent at the next lower grade, GS-07 level, in the Federal service or equivalent in other pay systems that equipped me with the particular knowledge, skills and abilities (KSAs) to perform successfully the duties of the GS-0675-08, Medical Records Technician (MRT) Coder - Experience is (l) any type of work that demonstrates your ability to ! perform the work of the position, (2) experience that provided a familiarity with the subject matter or processes of the broad subject area of the occupation. Experience must include the ability to perform multiple tasks in a medical environment that require advanced medical terminology, extensive medical coding and medical record documentation; accurately assign specific codes for the type of care provided; comply with VA and Federal reimbursement requirements; capture data programs, and adheres to coding practices, guidelines, and conventions when choosing the most appropriate diagnosis, operation, procedure, ancillary, or Evaluation Management.
2 . Medical Record Technician (Medical Coder) Positions. This is considered to be the full performance level for this assignment. Certification at this grade level is highly desired. In addition, the candidate must demonstrate the following KSAs:
a . Demonstrated Knowledge, Skills, and Abilities
( 1 ) Ability! to analyze the medical record to identify all pertinent diagnoses and ! procedures for coding, and to evaluate the adequacy of the documentation. This includes the ability to read and understand the content of the medical record, the terminology, the significance of the comments, and the disease process/pathophysiology of the patient;
( 2 ) Skill in reviewing medical record documentation and assigning current versions of the classification systems required in the current position, such as ICD and CPT; and
( 3 ) Advanced knowledge of the full scope of coding and abstracting including inpatient discharges, surgical cases, diagnostic studies and procedures, outpatient encounters, and inpatient professional fees for a highly diversified range of specialties and subspecialties, such as orthopedics, neurosurgery, cardiology, gastroenterology, plastic surgery, spinal cord injury, blind rehabilitation, anesthesia, acute and long term psychiatry including addiction treatment, hospice, ambulatory surgery, and other types of care.
